Reporting a correction

Include the center name, city, and CMS Certification Number (CCN, shown on every center page), plus what is wrong. We verify against the latest CMS release before changing anything.

Dialysis providers

Listings cannot be removed while a center is Medicare-certified, and quality data cannot be edited by us. Address, phone and service updates should be made in EQRS (ESRD Quality Reporting System); we pick them up at the next CMS data refresh.
